Abstrak
Abstract Background: Hallucinations are disorders of sensory perception of an object a sign of external stimulation, this perception disturbance includes all senses. Hallucinations are one of the symptoms of mental disorders (schizophrenia) that experience sensory changes in perception and feel a false sensation or without the actual stimulus in the form of hearing, vision, taste, touch, or smell (Yusuf, et al. 2015).Based on the research of dr. H. Moch Ansari Saleh Banjarmasin hospital, patients with cases of sensory perception disorder: Hallucinations became the most prevalent case in 2016 as many as 31 cases, and in 2017 as many as 50 cases, and the prevalence continued to increase in 2018 to 67 cases, From the results of research in the field, Clients look to pay attention to certain angles, speak for themselves or speak for themselves, The risk of violent behavior which is a result of unresolved hallucinations which risks injuring oneself, others and the environment. This is because the patient is under his hallucinations asking him to do something outside his consciousness, the intervention is by Implementation Strategy (IS) is an action taken so that the client recognizes its hallucinations, and trains or controls hallucinations on the client so that he can move as usual without hearing any whispers, black shadows that actually do not exist, but before being given SP the nurse first to establish relationships mutual trust (BHSP) with clients. Implementation standard (IS) consists of IS 1, namely how to rebuke and recognize the contents of hallucinations, IS 2 namely using drugs regularly, IS 3 which is conversing with other people, IS 4 is doing scheduled activities. Objective: Report the results of nursing care to clients with Sensory Perception Disorder: Hallucination in the psychiatric Ward. includes assessment, diagnosis, intervention, implementation, and evaluation. Method: This scientific paper is made using descriptive methods with a case study approach, in making use of methods that are collecting data, using data analysis and drawing data conclusions. Results: After nursing action, the results of the Sensory Perception Disorder: Hallucination can be resolved. Conclusion: The application of Implementation standard (SP) hallucination is very helpful and can restore the condition of client to be better.
